Questionnaire for students and families
This questionnaire is for all students and families of students who attend a BC K-12 public school.
It is important for our investigation to hear directly from students and their families. We welcome responses from everyone who has experienced exclusion from school.
- If you/your child are not currently attending school, please indicate the last grade you/your child attended school and what school district they were enrolled in.
- If you have more than one child who has been excluded from school, please fill out a questionnaire for each child.

Is my participation confidential?
Yes. Your responses are confidential. We will not share your name, your identity, or the identity or any students you mention wit the Ministry of Education, any school district, or anyone else. If we include information from your responses in our public report, it will be anonymous – no one will know who shared it.
Under section 16 of the Ombudsperson Act, you are also protected from retaliation for participating in our investigation. If you believe you have faced negative consequences because you took part in this survey or shared information with us, please contact us.
By completing this survey, you agree to let us collect and use your responses for this investigation. Again, all information will be anonymous. You can withdraw your consent at any time by email or call 1-800-567-3247.
How long will it take?
