Use the secure form below to submit a complaint to the B.C. Office of the Ombudsperson.

Please note: If you are a current or former employee of a government ministry or independent office of the legislature and want to report wrongdoing, please use the Report of Wrongdoing Form.

The online complaint form is designed to be a brief summary of your complaint. For a more detailed submission please print off one of our other forms and submit your information by mail or fax to us – or call 1-800-567-3247.

After you submit your complaint, our staff will review it and respond to you as soon as we are able to do so.

  • For the larger text boxes, please separate paragraphs by pressing the enter or return key.
  • When complete, please save or print a copy of your complaint for your records.
The Ombudsperson Office hours of work are Monday to Friday, 8:30am to 4:30pm. Please indicate if you have a preference for the best time for us to call you?
Is there anything else you think we need to know to follow up with you (e.g., limitations you may have in communicating with us, times or dates you may be unavailable, or privacy concerns)?
Give us the name of the organization you are complaining about. (Please be as exact as you can be.)
Give us the names of the people you have dealt with there. Give us any job titles, phone numbers or business addresses that you have for them.
Tell us your complaint. Tell us what you have done to try to solve the problem yourself. (Please give us any file or reference numbers and the dates these things happened.)
Why do you believe the organization's actions are unfair?
If yes, when was the last appeal or review and what was the result?
What do you want to happen? Describe the result or outcome.
If you consider the matter urgent, explain why.
